Dual Enrollment Transcripts for Graduated Seniors

Students who took a dual enrollment course at CHS and are not attending the University of Memphis must have the U of M transcript mailed to the college they will be attending. This is the only way your college will award the college credit.
You can request a transcript in a couple of different ways.
1. Online through TranscriptsPlus. Use Transcript Ordering Services to complete your request. Since you are not being authenticated through the portal, you will be asked to complete and submit a FERPA release form before you can complete a transcript request. FERPA regulations require that you personally authorize the release of your transcript to a third party. The fee is $2.85
2. In Person at the Registrar’s Office Front Counter (same day service available). Free
3. Postal mail using the Off-Campus Transcript Request Form.
Questions? Contact the University of Memphis or Courtney Wall, CHS Dual Enrollment Coordinator, at

Exam Check Out Permission Slip

Students must have the attached permission slip signed and in hand each day when they leave campus. These students may exit by the attendance office doors or the exit by the varsity gym. There will be administrators or designated teachers taking the permission slips as the students exit.

If a student does not have this form in hand, they will not be permitted to leave unless a parent comes to the attendance office and signs them out. Students will need a separate slip for each day.
