Sponsors/Coaches submit student rosters and an itinerary when requesting a field trip or athletic trip through MLP.
In addition, if the students will be traveling in private vehicles, these documents must also be provided and filed in principal's/building AD office:
1. Valid driver’s license(s) of driver(s) of vehicle(s) which have been scanned through Raptor
2. Copy of current insurance indicating liability limits of at least the TN state minimums of
25,000 (bodily injury), 50,000 (aggregate), and 15,000 (property damage)
3. Permission slips from parents/guardians verifying knowledge of driver with whom students will be riding
Groups can have all potential parent drivers complete the private vehicle form with required documents at the beginning of their season or the year. This will suffice for the remainder of that season/year.
