Medication Policies/Nurse

Parents and guardians,

Please know that Collierville Schools Policy does not permit students to possess any medication on school campus without physician orders and a completed Medication Authorization form and/or Medication Self-Administration Documentation form. Both documents are to be completed by a physician and kept on file in the nursing clinic. In addition, these documents must be updated and resubmitted each school year.   You can review the Drug Free Schools Policy, the Zero Tolerance Policy, and the Collierville Schools Medicine Policy by viewing the attachments on the right side of this page.
